FSA asks for the LFP application (Form CCC-853), an acreage report (FSA-578), proof of the grazing land you own or cash-lease, your livestock inventory, and the usual eligibility and payment forms.
FSA's LFP page lists these forms:
- CCC-853, Livestock Forage Disaster Program Application
- FSA-578, Report of Acreage (your grazing acres by pasture type)
- AD-2047, Customer Data Worksheet
- CCC-901, Member Information for Legal Entities, if you farm as an entity
- CCC-902I or CCC-902E, Farm Operating Plan, if applicable
- CCC-941, Average Adjusted Gross Income certification
- AD-1026, conservation compliance certification
- SF-3881, direct deposit form
FSA also asks for supporting proof. That means proof of grazing losses as of the drought date, grazing leases or permits, proof of land ownership, and livestock inventory information.
Inventory proof is where applications slow down. Sale barn tickets, purchase receipts, vet and brand records, and a dated head count all help show how many animals you were grazing.
Leases matter too. LFP pays on land you own or cash-lease. Share leases and the landlord's role can change who applies, so bring the lease and ask.
Many of these forms may already be on file if you've done business with FSA before. Your county office will tell you what's missing. Cownty never asks for any of these records and never files anything for you.
